About The Position

Landmark Hospital of Columbia has an exciting opportunity for a seasoned hospital-based social worker/discharge planner to join our dedicated and professional team. Landmark Hospitals are committed to providing the highest level of customer service and quality of care to those we serve. The Discharge Planner provides discharge planning and coordination for a group of patients in support of the social work and case management departments. Works directly with patients, families, and referral/community resources to address the various needs and barriers surrounding patient discharge planning (financial, clinical, psycho-social, logistical, etc.). This position also supports communication with external agencies to provide referrals and ensure a smooth transition from the LTACH acute hospital to the post-hospital setting. Works closely with admissions coordinators, RN Case Managers, and clinical teams to refine discharge plans and options. The Discharge Planner performs a variety of tasks related to coordinating care. A high degree of creativity and initiative is required to ensure that patients' unique needs are met in the most appropriate, timely, and cost-effective manner. The ideal candidate must be highly detail-oriented and goal-oriented, with superior analytic, critical-thinking, and time-management skills. They must have a positive, empathetic personality and the ability to work effectively with others as part of a multidisciplinary team in a complex care environment. Excellent communication and interpersonal skills are vital to the success of the Discharge Planner, who must also consistently display a professional attitude when faced with challenging cases.

Requirements

  • 2-year associate degree in a health-related field or bachelor's (Social Work preferred) or clinical license (such as LPN, RN, RT, PT, OT, or similar)
  • Minimum of 1 year of experience with patient care and interaction, preferably in an LTACH or Acute care hospital setting
  • Ability to develop and maintain positive working relationships
  • Strong computer skills preferred
