About The Position
For over 60 years, St. Anne Home, a ministry of the Felician Sisters of North America, has been dedicated to the continued healing mission of Jesus Christ by providing a continuum of care and supportive services to adults, their families and their caregivers. We are seeking to hire a Social Worker Designee.
Requirements
- Graduate of an accredited Bachelor of Arts program in psychology or sociology.
- Minimum of 1 year of Long-Term Care experience. Preferably in PA.
- Minimum of 1 year experience in the social services field.
- Awareness and knowledge of social service resources in the community.
- Criminal background check current within one year.
- Excellent oral and written communication skills.
Responsibilities
- Support the Director of Social Services and interdisciplinary teams.
- Help residents and families navigate psychological needs and coordinating services.
- Assist with documentation, assessments, care planning, and communication.
