About The Position

We are now hiring a Social Services Designee to be the newest member of our awesome team! Under the direction of the Social Services Director, our Social Services Designee will assist each resident and family in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations, and our policies and procedures.

Requirements

High school diploma or equivalent
Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals
Ability to organize, prioritize, and meet deadlines
Ability to maintain confidentiality and be discreet
Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice To Haves

Experience with geriatric population
AA or Bachelor's degree in a related field
Creative problem-solving skills
Strong communication skills
Sufficient computer skills to manage records and documentation

Responsibilities

Assist each resident and family in adjusting to placement, illness, and the plan of care
Help residents and families attain the highest practicable level of functioning in accordance with current federal and state regulations, and our policies and procedures
Assist residents and families with wishes regarding treatment
Help with their transition and adjustment to the facility
Participate in resident and family council meetings as requested
Support Admissions
Assist with discharge planning services
