The Social Worker/Counselor is an active member of the patient's interdisciplinary treatment team and is responsible for ensuring that the medically related emotional and social needs of patients are met and maintained on an individual basis in accordance with organizational policies and procedures, as well as applicable federal, state, and local regulations and standards. Responsibilities include conducting initial psychosocial assessments, developing and updating treatment plans, facilitating psychoeducational and process groups (including Mindability: Roadmap to Peace of Mind), participating in treatment team meetings, coordinating care with outpatient providers and community resources, discharge planning, and maintaining accurate and timely documentation within the electronic medical record (EMR).
