About The Position

Willow Rock is a 16-bed Psychiatric Health Facility in Alameda County for adolescents 12-17 years old who are experiencing acute psychiatric symptoms. Under supervision of a licensed clinician, the Social Work Clinician I provides general casework services to members served and natural supports. This involves person-centered recovery planning and collaborating with other services and agencies.

Requirements

  • A Bachelor’s Degree in Social Work or psychology from an accredited college or university
  • Demonstrated experience with behavioral health services
  • Must be at least 18 years of age
  • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
  • All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification.
  • If the position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment.
  • Ability to work effectively as a member of a multidisciplinary team
  • Thorough understanding of the rights of members served
  • Understanding of the conservatorship process
  • Knowledge of basic medical and psychiatric terminology

Responsibilities

  • Identifies and plans for psychosocial needs of members served
  • Works with members served and multidisciplinary treatment teams to design, implement, and evaluate the Recovery Plan
  • Provides education to members served and family members regarding mental health conditions and treatment options
  • Has responsibility discharge planning, including: Assessing readiness for discharge of members served in coordination with the recovery/treatment/service team
  • Making appropriate referrals for placement, case management, and aftercare services, as assigned
  • Coordinating with the Business Office to ensure that benefits and finances of members served are in order, as assigned
  • Developing and maintaining an awareness of community resources
  • Assures that members served know and understand their legal status and rights; tracks legal status on all assigned members served
  • Provides consultation to members served and their families regarding issues arising from the admission, discharge,and/or aftercare plan
  • If employed at Inpatient program, must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
  • If employed at Outpatient program, must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
