Requirements
- A bachelor's degree in Mental Health or related field
- Case Management experience
- Familiarity with medical/MH terminology/diagnosis
- Knowledge of accessing services in community
Nice To Haves
- Experience in a correctional health care setting
- Licensing preferred, not required.
- Baseline understanding the medical parole process preferred, not required.
Responsibilities
- Meets with all identified patients (pregnant and post-partum) to provide education related to the CTH set curriculum.
- Obtains all necessary consents to release information.
- Tracks all court dates.
- May assist with finding appropriate housing/residential programs for patients upon release.
- Coordinates all discharge planning related aftercare (medical, dental, vision, mental health, etc.).
- Provides Caregiving Affidavit and reviews Affidavit and all pertinent information.
- Documents all patient contact.
- Sends out a weekly census.
