About The Position

At NewYork Presbyterian Hudson Valley Hospital, talented Social Workers lead, learn, and educate in a department like no other. You will manage complex cases and serve as a strong and knowledgeable liaison between acute care and critical care teams, coordinating the efforts of all disciplines in support of patients and their families. Direct all aspects of inpatient care, ensuring optimal length of stay, anticipate outpatient service needs, and set follow-up appointments, consultations as needed, and other components necessary in the discharge planning process. This is a full time day shift position at NewYork-Presbyterian Hudson Valley Hospital. Position is primarily in Maternal Child Health but also occasionally may float to other units as needed.

Requirements

  • Master’s Degree in Social Work (MSW)
  • New York State License as Licensed Masters of Social Work (LMSW)
  • Ability to function independently
  • Ability to communicate effectively with administrative and professional staff and with people of diverse backgrounds and education
  • Ability to work with a variety of people and make decisions concerning issues involving discharge planning, home health situations and available community services
  • Must possess good understanding of Managed Care/HMO requirements, laws and regulations, Medicare/Medicaid requirements, policy and procedure, and community health, welfare and social agencies policies and procedures

Nice To Haves

  • Minimum two years’ experience in Care Coordination
  • Experience in Maternal Child Health
