About The Position
This role involves assessing patient needs and coordinating comprehensive case management plans from admission through discharge. The Social Worker will develop care plans that address complex physical, psychological, and social support requirements. A key aspect of the role is advocating for patient rights, preferences, and access to necessary emotional, financial, and social resources. The position also requires maintaining communication with leadership to implement care plans and monitor patient progress, including observation, readmissions, and length of stay.
Requirements
- Bachelor's degree in Social Work required.
- Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
- ACM also accepted.
- Certified Social Worker preferred.
- 3 years of experience preferred.
Responsibilities
- Assess needs and coordinate the interdisciplinary case management plan from admission to discharge
- Develop care plans considering complex physical, psychological, and social work support needs
- Advocate for patient rights, preferences, and emotional, financial, and social resources
- Maintain leadership dialog to implement care plans and monitor observation, readmissions, and length of stay
