About The Position

The Social Worker works as a member of the interdisciplinary team providing social work and discharge planning services and expertise to our Inpatient Cancer/Palliative Care Unit. The Social Worker makes decisions and accepts legal responsibilities for all aspects of professional practice. Role models both professional and organizational core competencies.

Requirements

  • Master’s degree in social work from an accredited university.
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • 3–5 years of relevant social work experience in an acute care or hospital setting.
  • Training or experience in grief and loss required.
  • Strong knowledge of psychosocial assessment, counselling, and conflict resolution.
  • Demonstrated ability to collaborate effectively with patients, families, healthcare teams, and community partners.
  • Excellent organizational, problem-solving, and communication skills.
  • Flexibility and ability to work under pressure in a fast-paced healthcare environment.
  • Regular and reliable attendance is an expected requirement of this position.

Nice To Haves

  • Membership in the Ontario Association of Social Workers (OASW) is preferred.
  • Experience in oncology, palliative care, or crisis intervention as an asset.

Responsibilities

  • Deliver psychosocial assessments and provide counselling, emotional support, crisis intervention, and grief support to patients and families.
  • Support patients and caregivers in coping with the effects of illness and treatment, including family, financial, and social challenges.
  • Facilitate discharge planning and connect patients and families with appropriate community resources and services.
  • Collaborate with physicians, nurses, and other interprofessional team members to ensure coordinated, patient-centered care.
  • Advocate on behalf of patients and families within the healthcare system and community.
  • Provide consultation and education to healthcare teams regarding psychosocial issues and available resources.
  • Maintain accurate documentation and statistics in compliance with hospital standards.
  • Participate in program development, hospital committees, and quality improvement initiatives.
