About The Position

St. Camillus is seeking a compassionate and skilled Social Worker (CSW) to join our Assisted Living team. This role plays a vital part in supporting residents and their families through transitions, care planning, advocacy, and ensuring a high quality of life in accordance with our mission and values.

Requirements

  • Bachelor’s degree in Social Work or a related human services field (accredited program).
  • Current Social Worker License (CSW) in the State of Wisconsin.
  • Minimum of 1 year of experience in a social work or leadership role within a CBRF or long-term care setting.
  • CBRF Certification (or ability to obtain within 6 months of hire).
  • Strong communication skills (written and verbal) with professionalism and discretion.
  • Proven ability to work independently and manage multiple priorities.
  • Excellent interpersonal, problem-solving, and conflict resolution skills.
  • Knowledge of community resources and how to access them.
  • Basic computer proficiency.

Responsibilities

  • Facilitate smooth transitions for residents entering assisted living from long-term care or independent living.
  • Develop and maintain social histories, care plans, and required assessments (e.g., mental status exams, depression scales).
  • Provide psycho-social interventions and evaluate their effectiveness.
  • Serve as a resident advocate, addressing concerns and resolving complaints.
  • Conduct interdisciplinary service plan meetings and coordinate care with other departments.
  • Educate residents, families, and staff on resident rights and available resources.
  • Assist with guardianship, Power of Attorney documentation, and related processes.
  • Coordinate admissions, discharges, and orientation processes for new residents and families.
  • Ensure compliance with CBRF regulations and organizational policies.
  • Maintain communication with residents, families, and healthcare teams to support individualized needs.
  • Support insurance-related matters and connect residents with community resources.
  • Participate in on-call rotation and provide backup support to Skilled Nursing Social Workers.
