Provides professional social work services through the assessment and resolution of problems and issues that impact overall health and wellbeing, including discharge planning. This role is part of the Hospice department and involves utilizing supportive counseling techniques, providing crisis interventions, and facilitating therapeutic, support, and education groups as needed to assist patients in developing coping strategies when lifestyle changes occur due to illness/disability. The position also involves collaborating with community boards, staying updated on relevant laws, and adhering to ethical codes. Essential duties include completing psycho-social assessments, facilitating Advance Directive forms, Pre-admission Screening forms, and verifying guardianship appropriateness. The role also involves making referrals to Medicaid waiver programs, observing and reporting indicators of abuse and neglect, acting as a patient advocate, mediator, and facilitator, and involving patients/families in decision-making. Accurate and timely documentation, assisting with new staff/student orientation, monitoring length of stay, and collaborating with the interdisciplinary team are also key responsibilities.
