About The Position

Within the context of a patient, client and family centred model of care, and in accordance with the mission, vision, values, and strategic directions of Providence Health Care, promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff. Reporting to the designated Social Work Site Leader, working under the overall direction of the Professional Practice Leader and Patient Resident Care Manager, and receiving guidance from the Research, Education and Practice Coordinator, the Social Worker, Brief Intervention Clinic provides advanced therapeutic clinical services to patients through complex assessments, individual and group therapy and education for adults who are being assessed, or receiving treatment for mental health. The Brief Intervention Clinic is a comprehensive service offering inpatient treatment services to adults throughout the Province of British Columbia. Working within a systems and structural theory-based model, the Social Worker coordinates care and treatment for a designated caseload. Liaises with professionals and teams throughout the province to assist with clinical treatment challenges and to provide education. Provides leadership within the team and the program and utilizes skills in dialectical and cognitive behaviour therapies to teach internal/external trainees from various disciplines such as psychiatric residents and fellows and other interdisciplinary students including nursing and social work students. Participates in program development and research activities, and makes presentations locally, provincially, nationally and internationally at conferences. Consults with and provides consultation to the BIC team, inpatient care teams and relevant community agencies on Mental Health practices as well as legislative issues including FIPPA, Health Care Consent Act, Mental Health Act and Adult Guardianship Act.The Social Worker has a central role in liaising, and sharing information, with community agencies and resources such as government agencies, hospitals, and community mental health services and charitable organizations around the province. The Social Worker functions in an interprofessional environment and is responsible for fostering a positive treatment milieu, policies and routines that are attuned to cultural, religious and ethnic diversity.

Requirements

Master's Degree in Social Work.
Completion of post-masters training in areas of Dialectical Behaviour Therapy (DBT) or Cognitive Behavior Therapy (CBT) or related equivalency.
Minimum of four (4) years experience specifically providing DBT or CBT in a related mental health setting or an equivalent combination of education, training and experience.
Current full registration with the British Columbia College of Social Workers (with Clinical Registration preferred).
Demonstrated knowledge of mental health and substance use issues and related evidence-based best treatment practices.
Comprehensive knowledge of psychiatric diagnoses and professional practice guidelines for the treatment of various disorders, including complex concurrent disorders.
Demonstrated knowledge and experience of relevant treatment modalities and interventions such as DBT, CBT, Solution-Focused Therapy, Brief Intervention Therapy, Crisis Intervention and motivational interviewing.
Demonstrated ability to communicate and work effectively with clients, families, interprofessional teams, community agency partners and academic/research institutions.
Demonstrated understanding of the social determinants of health.
Demonstrated ability to conduct comprehensive psychosocial assessments that include mental health status exams, and mental health and substance use standardized frameworks and tools.
Ability to plan, implement and evaluate practice interventions including individual, couple, family and group, based on psychosocial assessments.
Ability to make appropriate and effective referrals that reflect in-depth knowledge of community resources and strong collaborative skills in working with other community partners.
Knowledge of the scope of practice of other health care disciplines and the ability to work independently and collaboratively within an inter-professional team.
Ability to work effectively within a culturally diverse environment and to practice cultural safety and humility.
Demonstrated ability to organize and prioritize workload in a complex and demanding environment.
Demonstrated comprehensive knowledge of regulations, policies, procedures, and other statutory duties related to relevant legislation such as the Mental Health Act, Adult Guardianship Act, and the Freedom of Information Act.
Ability to educate and provide consultation to clients, families and healthcare professionals that includes the development, coordination and delivery of in-services, presentations, and psycho-educational and support groups.
Proficiency in the use of personal computers, including related word processing and database management software applications.
Physical ability to carry out the duties of the position.

Responsibilities

Evaluates, develops and implements treatment practices, guidelines and protocols within the team to promote excellence in social work practice and mental health treatment protocols.
Participates in complex discharge planning with community agencies and assists in coordinating treatment follow-up with other team members and community partners through methods such as researching available services, liaising with service and program providers, advocating for services, and providing advice and consultation to colleagues.
Utilizes skills in teleconferencing to include health care professionals in care planning with patients and families around the province.
Establishes and maintains effective collaborative and constructive liaison relationships with community mental health providers, social workers, academic institutions, hospitals and other agencies, in order to coordinate services across the continuum of mental health services.
Provides advanced therapeutic clinical counseling services to clients, families and groups in designated program area by exploring problems and personal concerns, treatment objectives, and emotional responses; facilitating change in coping style, behaviour, attitude and feelings; participating with clients, families, interdisciplinary team members and community partners to coordinate services, facilitate meetings and make referrals to appropriate resources.
Delivers and coordinates therapeutic and psycho-educational groups for specific client populations.
Conducts comprehensive social work psycho social assessments in designated program area using methods such as interviewing the client and/or family to obtain relevant information, gathering social data regarding the client's family, supports, environment, culture, living situation and financial status and interpreting the data to inform interventions and plans to enhance the client's and family's coping abilities.
Formulates and implements care and treatment plans through methods such as negotiating a process for achieving goals, collaborating with members of the inter-professional team(s) in both inpatient and outpatient settings and evaluating goals while maintaining respect for client dignity, culture, self-determination and independence.
Maintains client records through electronic methods such as documenting client assessments, charting client information and regular progress notes and reports in accordance with established standards, policies and procedures to meet regulatory requirements and in accordance with the Freedom of Information Act.
Provides in person and, through conference calls and videoconferencing, consultation to mental health providers around the province on complex cases and therapeutic challenges.
Takes leadership in program planning and development of clinical practice guidelines as part of a multi-disciplinary team.
Identifies, promotes and leads program planning strategies to enhance system processes and resources for patients.
Takes leadership in keeping up-to-date on research and best practices in treatment methods in the mental health field and specific to social work practice.
Maintains knowledge of community resources locally, nationally and internationally in the field of mental health.
Initiates and participates in psycho social research.
Implements appropriate evidence-based research outcomes into strategic planning and professional practice.
Provides clinical consultation to the BIC team, care teams across PHC and relevant community agencies regarding vulnerable adult protection concerns, release of information and health care decision-making.
Advocates for the patient, family or caregiver to address gaps in resources within the hospital and the community.
Assists patients and families with referrals to community agencies as needed.
Provides presentations locally, provincially and nationally at conferences, as well as provides education to PHC interdisciplinary audiences.
Provides practicum supervision and teaching to master's level Social Work students regarding the social work role, individual and group therapy.
Provides consultation and teaching on a case-by-case basis in individual and group therapy for psychiatric residents and fellows as well as community professionals who come to the program for training.
Participates in relevant committee work within the program, the hospital and the community.
Performs administrative tasks such as writing reports, maintaining statistics and updating health records.
Performs other related duties as assigned.
