About The Position

To provide assertive Community Outreach and actively participate on a multidisciplinary team to provide direct client services for people who are living with serious, complex mental illness, and who require long-term support. Reporting under the general direction of the Clinical Manager, Psychosocial Rehabilitation.

Requirements

  • Minimum of a four (4) year Bachelor’s Degree in Social Work, from an accredited university.
  • Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
  • Minimum three (3) years of recent, relevant experience in providing psychiatric assessment, treatment, monitoring and evaluation of patients in a community setting.
  • Minimum of one (1) year recent experience managing community treatment orders (CTO).
  • Experience in community-based, outpatient programs is required.
  • Demonstrated ability to practice in the areas of Psychotherapies with adults and families.
  • Demonstrated Health Care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV).
  • Demonstrated skills in Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and individual and group interventions.
  • Demonstrated knowledge and understanding of principles of recovery as it applies to mental health and addictions treatment.
  • Demonstrated ability to manage rapidly changing mental health states and psychiatric emergencies.
  • Demonstrated skills in a range of individual and group psychotherapies, as well as experiential therapies for the treatment of addictions & mental illness.
  • Demonstrated excellent judgment and proven analytical skills.
  • Demonstrate strong client and family service orientation.
  • Demonstrated knowledge of culturally safe practices.
  • Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  • Demonstrated superior interpersonal and communication skills, both written and verbal.
  • Effective time management and organizational skills with the ability to organize and prioritize as required.
  • Demonstrated discretion and maturity when handling confidential information.
  • Demonstrated commitment to the safety of co-workers and patients.
  • Successful Vulnerable Sector Check is required.
  • Ability to use tact and discretion in dealing with health care providers and patients.
  • Demonstrated ability to work effectively as a member of an interdisciplinary team.
  • Demonstrated commitment to ongoing professional development.
  • Demonstrated professionalism in dealing with confidential and sensitive issues.

Nice To Haves

  • Certification in Cognitive Behavioral Therapy, including forty (40) hours of theory and supervision hours, is preferred.
  • Additional training in Addictions and Counselling and Treatment is preferred.
  • Experience in community-based, outpatient programs is required.
  • Experience providing treatment in a group setting is preferred.

Responsibilities

  • Provide evidence-based treatment for individuals in the Psychosocial Rehabilitation programs.
  • Complete intake assessments.
  • Develop and implement intervention plans based on best practice within a team approach to minimize or eliminate identified impacts and problems.
  • Document assessments, treatment plans, progress notes, and outcomes in a timely manner.
  • Assist in and act as a resource for client and family education.
  • Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
  • Provide guidance, training and orientation for new staff and students.
  • Work collaboratively with community providers to provide wrap around care for complex clients.
  • Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  • Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  • Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Educate and promote health, safety and wellness in the work place.
  • Represent the department or program on various committees and in meetings as required.
  • Perform other duties as required.
