The Social Worker Assistant will identify psychosocial, financial, and environmental needs and provide care appropriate to adolescent, adult, and geriatric populations. This role involves making initial evaluation visits, re-evaluating patient needs, and developing a plan of care. The assistant will communicate significant findings to supervisors, physicians, and other care personnel, and participate in the implementation and updates of the plan of care to ensure quality and continuity of care, including proper discharge planning. They will also incorporate short and long-term goals and perform appropriate skilled services/interventions in accordance with accepted standards of practice, as certified by the patient's physician. Counseling and instructing patients and families regarding psychosocial needs is also a key function.
