Social Worker Apprentice - Case Management - PT - 1st Shift
About The Position
The Apprentice Social Worker Student works under a supervising licensed social worker serving in the role of preceptor. The responsibilities of the Apprentice Social Worker Student include assisting licensed social worker with resource management activities for inpatients based on identified patients. Helps to coordinate patient’s transition from hospital to home through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction.
Requirements
- Must have a Bachelor’s degree from an accredited university.
- Enrolled in Alabama A&M’s Master of Social Work program
- American Heart Association BLS Certification
