About The Position

Facilitates resource management activities for inpatients based on identified patients. Coordinates patient’s transition from hospital to home through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction. Provides psychosocial assessment, resources and intervention to hospitalized patients.

Requirements

  • BS degree in Social Work from an accredited college or university and current Alabama licensure
  • Excellent communication skills
  • Capable of working with a diverse population
  • Behave ethically and professionally
  • Must be able to collaborate well with others
  • Demonstrate emotional resilience and stability

Nice To Haves

  • Two years hospital experience preferred
  • Comparable combination of education and experience will be considered
  • Previous discharge planning or hospital based social service experience very beneficial
