This position will work with Gastroenterology clinics. The Social Worker will provide biopsychosocial assessments, clinical behavioral health diagnoses, treatment planning, evidence-based clinical interventions, and information and referrals to the community. They will interview patients and their social support systems to complete initial psychosocial evaluations and provide brief psychotherapy, supportive counseling, and education to assist patients in coping with stress related to hospitalization, disability, chronic/terminal illness. The role involves identifying, planning, and coordinating aspects of care, developing and revising plans of care in collaboration with a multidisciplinary team, and assessing the effectiveness of patient/family education. The Social Worker will advocate for patients and families, serve as a resource, intervene in crisis situations, and participate in the development of clinical pathways and population-based programs. They will also analyze and evaluate the impact of social work involvement on quality outcomes and implement strategies for improvement, and refer patients/families to external resources when appropriate. Accurate and timely documentation is required. Participation in clinical research and presentations may also be involved.
