About The Position
Woodcrest Rehabilitation & Residential, a facility located in Flushing, NY, is seeking a Full-time Social Worker (BSW or MSW). Long Term Care/ Subacute experience is preferred. The role involves attending and participating in interdisciplinary care plan meetings, completing MDS while developing and maintaining individual care plans, interviewing and assessing residents’ needs during the admission interview process, working with the team to coordinate discharges, and working with residents to maintain psychosocial well-being.
Requirements
- BSW or MSW degree.
