About The Position

The Cognitive Care Network Community Navigator is responsible for developing relationships with health care providers (primarily MD, DO, APRN) in designated Kansas counties and, in collaboration with the Director, advance the use of clinically indicated cognitive screening tools, enlist providers in the Cognitive Care Network, and advance research referrals. Once a provider is part of the Cognitive Care Network, the Community Navigator will serve as Care Navigator for patients referred from those practices for post diagnosis education and support as well addressing the needs of patients with care complications.

Requirements

  • Master of Social Work or related degree/clinical licensure.
  • Six (6) months of dementia care experience including direct work with individuals identifying with memory concerns, individuals diagnosed with dementia, or families of those diagnosed.
  • Six (6) months of experience in clinical research or community outreach.

Responsibilities

  • Establish and maintain relationships with providers and continually evaluate barriers to practice translation of cognitive health and dementia care protocols, Cognitive Care Network participation, and integration of research connections.
  • Inform and coach providers in use of cognitive screening tools and clinical application to patients.
  • Track number of provider contacts/status and referral data.
  • Counsel and educate patients and families post diagnosis disclosure.
  • Assess contributors to neuropsychiatric challenges associated with dementia and make recommendations to both families and clinical team.
  • Develop dementia management plans along with patients and families.
