About The Position

The Adolescent Partial Hospitalization Program (PHP) is seeking a licensed social worker to co-facilitate and facilitate therapeutic groups and provide direct support to adolescents ages 13–18 during program hours (9:00 a.m.–3:00 p.m.). This role involves working closely with youth in a structured, team-based setting to support emotional regulation, skill development, and overall treatment engagement throughout the day. The ideal candidate values collaborative care and enjoys building meaningful connections with adolescents in a group-based treatment environment.

Requirements

• Bachelor’s degree in Social Work or in a related field
• WI Social Worker license
• 1 year of experience in human services or health care

Responsibilities

• Assessing incoming referrals
• Scheduling appointments
• Coordinating patient care
• Providing or assisting with crisis intervention
• Meeting with patients
• Facilitating/co-facilitating groups
• Assisting with programming needs
• Act as a consultant and leader in comprehensive care throughout the system, while supporting organizational strategies and goals.
