About The Position

Social Work Services at the Mount Sinai Hospital is a large department with a long history, employing over 450 licensed social workers across various service areas. They are committed to total patient care, addressing physical, emotional, and social needs. The Mount Sinai Adolescent Health Center (MSAHC) focuses on breaking down barriers to healthcare for young people by providing comprehensive, integrated, and confidential services at no cost. MSAHC aims to advance adolescent and young adult health through clinical care, specialized training, research, and advocacy. The A28 Social Worker will serve at-risk adolescents and their families within an integrated medical and mental health primary care setting at MSAHC. This role involves providing short-term individual, family, and group psychotherapy, intensive case management, crisis intervention, and health education as part of an interdisciplinary team. The social worker will consult with medical providers on high-risk patients with complex needs and conduct clinical assessments. A key aspect of the role is developing innovative models to engage and retain adolescents, particularly those with trauma histories (e.g., sexual abuse, domestic violence, community violence, physical abuse).

Requirements

  • NYS LCSW or LMSW
  • Commitment to MSAHC’s mission, vision and approach to ensure a patient-centric culture which is collaborative and multidisciplinary
  • A passion for working with young people and a work ethic in which a commitment to understanding and solving the needs of each patient is a priority
  • A demonstrated commitment to diversity, equity, inclusion and belonging
  • Skillful in assessing patient needs in relationship to health and psychosocial functioning and establishing intervention accordingly
  • Significant experience working with low-income, inner-city, HIV positive, at-risk adolescents
  • Experience in delivering therapeutic interventions, such as preventative and risk-reduction models that reflect developmentally appropriate methods and materials in working with adolescents and young adults (i.e. TF-CBT, CBT, MI, DBT, EMDR, CPT, IPT-A)
  • Experience in providing individual therapy and group work
  • Excellent collaborative, clinical and communication skills
  • Excellent organizational and documentation skills
  • Skillful in collaborating with external resources and coordinating appropriate referrals
  • Working knowledge of state regulations related to child abuse, neglect, and other urgent care issues

Nice To Haves

  • Experience working with populations with complex trauma histories preferred
  • Experience working with LGBTQIA identified adolescents and young adults preferred
  • Ability to be flexible and use non traditional approaches in working with young adults preferred
  • Bilingual (English/Spanish) preferred

Responsibilities

  • As part of an interdisciplinary team, the social worker will actively engage in the assessment and treatment planning of patients referred from medical providers in Article 28 setting while providing short-term therapy as needed.
  • Develop therapeutic alliance and deliver therapeutic interventions while consulting closely with medical providers and child and adolescent psychiatrists who are working with patients.
  • Develop effective treatment plans and track treatment progress using validated instruments.
  • Participate in an interdisciplinary effort that fosters a cohesive culture across staff with improved communication and accountability while collaborating with all practices to ensure integration of the MSAHC integrative model of care.
  • Provide transgender and gender non-conforming (TGNC) youth with individual, and family psychotherapy, intensive case management, crisis intervention, and health education.
  • Provide assessments for gender dysphoria and hormone and surgery readiness if needed.
  • Provide counseling related to medical transition.
  • Ensure that the above functions are delivered within a culture that is youth-centric, high quality, integrated and collaborative as well as in accordance with Mount Sinai’s policies and procedures, New York State regulations and guidelines, and funder expectations.
