About The Position

ChristianaCare is hiring a full time Social Worker II for our Primary Care at Home program for New Castle County, DE. Do you want to make a meaningful impact in patients’ lives beyond traditional clinical settings? As a Primary Care at Home Social Worker, you will play a vital role in supporting patients with chronic and complex medical needs in the comfort of their own homes. This position provides compassionate counseling, care planning, and ongoing support while addressing social determinants of health and helping patients and families navigate challenges. Working closely with an interdisciplinary care team, you will promote patient stability and independence, support care transitions, and connect individuals to community resources and services that enhance quality of life. Requirements

Master’s degree in social work (MSW) from an accredited graduate school of Social Work

Nice To Haves

Minimum of 1-2 years of related work experience
Delaware Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
