About The Position

The Social Worker LMSW conducts psychosocial evaluations, care management services and discharge planning. Develops and implements patient treatment plans and participates in treatment team conferences. Relies on education, experience and judgment to accomplish job. Works under supervision required by regulation and licensing board. Creativity and some latitude is expected to complete responsibilities.

Requirements

  • Master's Degree
  • Current Louisiana State Board of Social Worker License (CSW or LMSW)
  • Proficient in the English language, verbal and written communications skills
  • Basic computer skills
  • Experience providing case management, social services, care coordination, or client advocacy.
  • Excellent communication, documentation and computer skills, interpersonal, organization, and problem-solving skills to work effectively with individuals, families, healthcare professionals, and community partners.
  • Ability to manage multiple cases, priorities, and deadlines independently.

Responsibilities

  • Conduct psychosocial assessments and gather relevant medical, social, and financial information.
  • Educate clients and families regarding the placement process, levels of care, and community resources.
  • Assist families in identifying appropriate nursing facilities (available levels of care) based on clinical, financial, geographic, and personal preferences.
  • Coordinate referrals and admissions to nursing homes and long-term care facilities.
  • Serve as a liaison between families, healthcare providers, nursing facilities, and community agencies, advocating for timely access to appropriate long-term care services.
  • Maintain regular communication throughout the placement process.
  • Provide individualized case management, education, advocacy, and ongoing support throughout the placement process.
  • Identify barriers to placement and develop solutions in collaboration with clients, families, and referral partners.
  • Coordinate communication among clients, family members, healthcare providers, nursing facilities, and community resources.
  • Assist with gathering required documentation and completing appropriate applications, referrals, and placement-related paperwork.
  • Follow up with clients and families to monitor progress, address barriers, and ensure continuity throughout the placement process.
  • Maintain accurate, timely, and confidential case records in accordance with organizational and regulatory requirements.
  • Develop and maintain strong referral relationships with physician offices, clinics, rehabilitation centers, home health agencies, hospice providers, and other healthcare organizations.
  • Identify opportunities to expand referral networks.
  • Maintain ongoing communication with referral sources and community partners.
  • Distribute marketing materials and education regarding placement services through presentations and educational programs.
  • Represent the organization at community events, health fairs, networking events, and professional meetings.
