About The Position
The Social Worker 2 role within the Emergency Psychiatric Access Team offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists individuals with various issues. We currently offer an opportunity for a Part time, Nights Social Worker to support patients with mental health issues.
Requirements
- Master's Degree from an accredited school of Social Work (Required)
- Computer skills (Required proficiency)
- Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
- Multi-tasks and prioritizes work (Required proficiency)
- Works autonomously (Required proficiency)
- Communicates effectively with persons of various backgrounds (Required proficiency)
- Licensed Social Worker (LSW) (Required)
Nice To Haves
- Hospital/Health Agency experience. (Preferred)
- Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility)

Responsibilities
- Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
- Provides age specific care to patients/families in all age groups.
- Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
- Develops plans with attention to individual patient and family goals.
- Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
- Communicates/collaborates with the work team using a multidisciplinary approach.
- Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
- Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.
- Performs other duties as assigned.
- Complies with all policies and standards.
- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
