HCBS social workers help coordinate services for at-risk older adults and people with disabilities so they can live in the community and avoid institutionalization. They assess risk and service needs, make referrals, and determine case outcomes under applicable regulations, with the aim of ensuring client safety. Social workers in the series provide case management to vulnerable adults and other individuals in local communities and institutions. Case management involves identifying individual needs and locating, coordinating, and monitoring social, habilitative, and medical services. Positions may focus on one service area or work across several areas, depending on community needs and agency mission. At this trainee level, incumbents work under close supervision while learning to determine client needs, develop solutions and case plans, and provide ongoing case management. Progression to the next level may occur after minimum qualifications and satisfactory performance are achieved and the appointing authority recommends advancement. ADSD describes its work environment as positive, inclusive, and diverse, valuing innovation, teamwork, and collaboration. Its mission is to empower individuals and their support systems by providing resources for disabilities and aging, connecting Nevadans to services and improving quality of life.
