Under the supervision of the Social Services Technical Manager, the PACE Social Services Supervisor oversees site-specific San Diego PACE Social Services provision, consistent with standards of social work and San Diego PACE, promoting and ensuring quality, efficiency and accessibility of Social Services to PACE participants and families. The Social Services Supervisor is responsible for leading social workers and case Managers in the provision of social work services and ensuring high quality standards within the Social Service Department. The Social Services Supervisor may, as needed, carry a caseload (panel) and will train social workers and case managers to effectuate social services, including planning, organizing, implementing and closing the loop for all social services needed for San Diego PACE participants and their families. The PACE Social Services Supervisor will oversee outcomes for the department and provide weekly quality and productivity reports to the Site Manager and Social Services Technical Manager. Responsibilities include but are not limited to: organization of daily workflow, supervision of social service employees and related client outcomes, of QAPI projects, assessment, auditing charts and managing employee performance, treatment(s), teaching and counseling to participant, caregiver or other appropriate representatives. Social Services is the liaison between the interdisciplinary team, caregiver representatives, and community agencies.
