About The Position

Reporting to the MSSP Supervising Care Manager, the Berrellesa Social Worker-Site Coordinator (SW/SC) provides services to the residents at the Berrellesa Palms housing development in Martinez. The SW/SC provides non‐clinical case management and referral services for residents. Services include securing the full range of social services as needed by individual residents. The SW/SC works as a member of the CiA team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to “create opportunities where people can learn, grow and age independently with dignity in community”.

Requirements

  • A Master’s degree in social work from an accredited college or university and one year’s experience as a Social Worker in a health related field, with demonstrated knowledge of case management, OR A Bachelor’s degree in social work & two (2) years’ experience as a Social Worker in a health related field, with demonstrated knowledge of case management.
  • Possess knowledge of community resources for the disabled and elderly.
  • Must have clean DMV record, reliable personal transportation, and auto insurance.
  • Must be able to communicate clearly both verbally and in writing.
  • Ability to work independently as well as in a team environment.
  • Being fully vaccinated means that an individual is at least two weeks past their final dose of an authorized COVID-19 vaccine regimen. Final candidates must be fully vaccinated as of their first day of employment.

Responsibilities

  • Provide non‐clinical case management and referral services for residents; including securing the full range of social services as needed by individual residents.
  • Maintain accurate and timely documentation on all resident demographics, issues, requests, statistics, incidents, interactions and outcomes, using Salesforce
  • Complete resident narratives daily and submit narratives to owners Resources for Community Development (RCD)/ CiA at the end of each month.
  • Complete tracking sheets daily to monitor where time is being allocated; forward weekly recap to CiA supervisor.
  • Educate residents on service availability, benefits and other application procedures, tenant rights, tools for healthy living and lease compliance.
  • Monitor, document and help residents prevent and manage cognitive, emotional, and physical health issues, including ADL limitations.
  • Conduct outreach and engagement, monitoring services provided.
  • Schedule and conduct home visits when required.
  • Establish constructive relationships with residents and their families, community agencies, volunteer organizations, and company staff, especially those in property management.
