The Social Work Services Program Consultant functions as a member of the Children’s Multidisciplinary Assessment Team (CMAT) and provides social work services and support to families and children served by the CMAT program. This role involves completing day-to-day administrative operations, reviewing CMAT referrals, communicating with stakeholders, completing psychosocial assessments, and supporting parent participation in the CMAT process. The consultant will participate with the team to determine client eligibility for services, engage in collaborative decision-making, and communicate relevant information and policies to CMAT members. They will encourage the placement of clients in the least restrictive setting, complete and disseminate CMAT staffing summaries, and document CMAT activities in the client's electronic medical record. Additionally, the role includes facilitating access to client reports and records, providing data from database systems, and participating in training and evaluation activities to improve CMAT functions and outcomes. The consultant will also ensure the completion of level of care training for CMAT members and perform other assigned duties.
