About The Position

The Social Services Consultant provides supportive services to family and care team members throughout the patient's assessment and treatment. Duties may include but are not limited to, meeting with patients, conducting needs assessment, providing referrals to community services.

Requirements

  • Bachelor degree in social work, counseling community health or related field plus one (1) year of experience in care and resource coordination for clients in medical settings.

Responsibilities

  • Conducts comprehensive psychosocial assessments to identify barriers to health, such as housing instability, food insecurity, transportation issues, financial concerns, caregiver stress, mental health needs, and substance use.
  • Provides brief interventions, counseling, crisis intervention, and supportive services for patients and families within the primary care setting.
  • Coordinates referrals to community resources, social service agencies, behavioral health providers, and specialty care.
  • Assists patients in accessing benefits and services, including Medicaid, Medicare, disability resources, and community assistance programs.
  • Collaborates with physicians, nurse practitioners, nurses, pharmacists, psychologists, and other members of the interdisciplinary care team to develop patient-centered care plans.
  • Participates in population health initiatives that improve health outcomes for patients with chronic diseases and complex medical and social needs.
  • Supports care transitions by helping patients navigate hospital discharges, follow-up appointments, and continuity of care.
  • Documents patient encounters and interventions in the electronic medical record while maintaining compliance with UAMS policies and HIPAA requirements.
  • Educates patients and families about coping strategies, disease management, and available community resources.
  • Participates in quality improvement projects, program development, and team meetings to enhance integrated primary care services.
  • May perform other duties as assigned.
