Join Neighborhood PACE! As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, you will work closely with participants and their caregivers to help them continue to live safely in their homes and communities as independently as possible. You will work in an integrated primary care and behavioral health setting to support participants in their homes and at one of our PACE centers. This is a full-time position to act as a case liaison and support for the Neighborhood PACE Social Work department. You will work with older adult participants and families to complete case management and housing tasks to maximize participant and caregiver satisfaction. An ideal candidate will be knowledgeable about community resources, be detail-oriented, able to multitask, and possess excellent communication and computer skills. Must hold a Bachelor’s degree in social work or related field or have two years of relevant work experience in a community setting. Bilingual (English/Spanish) preferred. Automobile required.
