About The Position

This role is responsible for promoting a safe, cooperative, and professional healthcare environment to ensure optimum patient care. The position involves adhering to all hospital policies and procedures, prioritizing customer service, and demonstrating standards of performance consistent with patient-centered excellence. The role also includes assisting the team in patient care delivery, performing related clerical duties, utilizing effective communication techniques, and understanding hospital policies and procedures. Additionally, it involves participating in educational and professional development, demonstrating age-specific competencies for adult and geriatric populations, and performing specific navigator responsibilities. Proficiency in EMR (EPIC) and email/telephone etiquette is also required.

Requirements

  • Master’s degree in social work (MSW) from an accredited program required.
  • Current state licensure (LMSW/LCSW or equivalent) preferred or required based on state regulations.
  • Experience working with children, adolescents, and families preferred.
  • Knowledge of pediatric medical conditions, child development, and behavioral health issues.
  • Strong crisis intervention, communication, and care coordination skills.
  • Experience in outpatient healthcare or pediatric settings.
  • Familiarity with community resources and insurance systems.
  • Ability to work effectively within multidisciplinary teams.
  • Two (2) years’ experience in outpatient settings.

Nice To Haves

Responsibilities

  • Promote a safe, cooperative and professional health care environment to ensure optimum patient care.
  • Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.
  • Makes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner, exercising courtesy and tact.
  • Effectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion.

  • Assist the Team in the delivery of the patient care considering age/development level, under the direction of the Professional staff.
  • Perform tasks as specified by Director of Care Transitions leadership team.
  • Maintains an orderly flow of patients.
  • Follows OSHA standards and Infection Control policies.
  • Seeks direction and supervision, when necessary, but functions independently where appropriate.
  • Correctly identifies all patients by dual identifier prior to performing any procedures.
  • Demonstrates flexibility with changing assignments due to staffing needs and adequately manages workload.
  • Performs related clerical duties to ensure continuity of patient care.
  • Utilizes computer technology in the work area.
  • Enter data accurately.
  • Discharges patients and make referrals as indicated by the team.
  • Maintains a safe environment for patients and report unsafe condition to the unit manager.
  • Utilizes communication technique effectively.
  • Demonstrates a professional caring, respectful approach with patients, families and co-workers.
  • Understands and respects cultural differences.
  • Communicates in a pleasant, courteous, respectful manner always.
  • Maintains confidentiality and privileged communication.
  • Uses effective communication to assist in directing patient flow.

  • Participate in staff meetings.

  • Age Specific Competencies - Adult and Geriatric Population.
  • Modifies approaches based on age-specific needs and responses to treatment.
  • Uses communication techniques that are age appropriate.
  • Specifics Navigators’ Responsibilities-
  • Provide information to outpatient substance programs.
  • Provide patients with adequate resources such as housing, food, and legal services. Based on the SDOH.
  • Referral to community agencies. Meals on wheels.
  • Referral to Health Home.
  • Call patients that were Discharged from the ED.
  • Asst inpatient Social Work staff without patient resources.
  • EMR knowledge and skills – EPIC.
  • Documents patient care navigator notes.
  • Email/telephone knowledge and etiquette.
  • Respond to incoming emails and calls in a timely matter.
  • Ensure out of office notifications are activated on scheduled days off.
  • Review emails and voicemails on a daily basis.
