About The Position

This position promotes patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources. It manages, supports, and develops comprehensive transitional care plans for patients with complex and psychosocial needs in inpatient and Emergency Department environments. The role promotes quality of care and effective transition planning by collaborating with service team members, physicians, patients, external agencies, and families. The schedule is weekends on Saturday and Sunday, plus either Friday or Monday, with a shift option of 7:00 a.m.–7:30 p.m. or 8:00 a.m.–8:30 p.m.

Requirements

Master’s Degree in Social Work from an accredited social work program.
At least 1 year of experience in an acute or post-acute setting.

Nice To Haves

LMSW - Licensed Masters Social Work.
Accredited Case Manager (ACM).
CCM - Certified Case Manager.
