Requirements

  • Master’s degree in social work.
  • Massachusetts LICSW licensure.
  • 5 years of previous LICSW Social Worker related experience.
  • Demonstrated Leadership ability.
  • Demonstrated ability in psychosocial assessment, knowledge of community resources, crisis intervention skills, ability in discharge planning, and ability to work with a multidisciplinary team.
  • Knowledge of biopsychosocial aspects in the Managed Care/health care field.
  • Excellent organizational and communications skills.

Responsibilities

  • Identifies patient needs and develop quality social work programs to meet these needs. Ensure the delivery of quality social work for inpatients and outpatients. Consultation with medical, nursing, administrative staff, and community agencies. Manage caseload and staff hours to suit the needs of patients.
  • Develops and maintains quality assurance records and evaluate the provision of services and reporting methods of the department. Ensures that the department is represented at conferences and working committees.
  • Works collaboratively with manager, Information Services, and others to continuously review and develop technology resources that improve workflow.
  • Acts as liaison to clinical departments, physicians, and other customers. Serves as the primary contact for social work issues.
  • Maintains a collaborative relationship with the Director of Care Coordination and ensures that the Social Workers and Care Coordinators work together in the best interests of patients.
  • Collaboratively facilitates the care of assigned patients throughout the continuum of care.
  • Educates the patient, family and care team regarding resource options and facilitates decision making, initiates plan(s), and secures resources for assigned patients on an ongoing basis in complex cases.
  • Complies with established departmental policies, procedures and objectives including cross campus coverage.
  • Identifies objectives and sets performance goals to evaluate the performance of social work staff and the program.
  • Directs and supervises assigned personnel including performance evaluations, scheduling, orientation, and training. Makes recommendations on employee hires, transfers, promotions, salary changes, discipline, terminations, and similar actions. Resolves grievances and other personnel problems within position responsibilities.
  • Develops and recommends the budgets for the areas managed. Manages activities to assure financial goals are met.
  • Coordinates the assignment of tasks and helps resolve technical and operational problems. Evaluates the impact of solutions to ensure goals are achieved.
  • Provides effective direction, guidance, and leadership over the staff for effective teamwork and motivation and fosters the effective integration of efforts with system-wide initiatives.
  • Encourages and supports diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, respect, tolerance, civility and acceptance toward all employees, patients and visitors.
  • Integrates diversity into departmental objectives, such as hiring, promotions, training, vendor selections, etc.
  • Participates in performance improvement initiatives and demonstrates the use of quality improvement in daily operations.
  • Ensures compliance with regulatory agencies such as Joint Commission, DPH, etc. Develops and maintains procedures necessary to meet regulatory requirements.
  • Ensures that department complies with hospital established policies, quality assurance programs, safety, and infection control policies and procedures.
  • Ensures adequate equipment and supplies for department.
  • Develops and maintains established departmental policies, procedures, and objectives.
  • Ensures compliance to all health and safety regulations and requirements.
  • Maintains, regular, reliable, and predictable attendance.
  • Performs similar or related duties as required or directed.
