Baptist Health is seeking a Social Work Case Manager to intervene with patients and families regarding psychosocial needs, providing assistance and support in coping with chronic, acute, or terminal illness. Services include consultation, discharge planning, education, counseling, advocacy, and referrals to meet emotional, social, and financial aspects of care. The role involves crisis intervention, coordinating discharge plans, and serving as a liaison between patients and community resources. The Case Manager identifies patient and family psychosocial and environmental needs related to their condition and treatment, working collaboratively with an interdisciplinary team to develop comprehensive discharge plans. They make appropriate referrals to community services, provide inpatient emotional support and counseling, crisis intervention, and bereavement services. Additionally, they assess and intervene in situations involving child or elder abuse/neglect and domestic violence, providing care to all patient groups: neonatal, infant, pediatric, adolescent, and adults.
