About The Position

Home & Health Care Management is seeking bachelor’s prepared Social Workers (or related fields like Gerontology) to join a Care Management Team (CMT). The CMT consists of Master’s or Bachelor’s prepared Social Workers and Registered Nurses. This role involves providing comprehensive case management services to older adults, children, and individuals with disabilities. The agency specializes in in-home care and case management, utilizing Medi-Cal, Medicare, and targeted health programs to find solutions for clients. The ideal candidate is detail-oriented, computer literate, a problem-solver knowledgeable about community resources, and passionate about improving the lives of individuals. This position is based in our Sacramento office and requires travel to client homes within Sacramento County and surrounding areas.

Requirements

  • Master's degree or Bachelor’s degree in Social Work or another related field
  • Skilled in operating a personal computer utilizing a variety of standard software
  • Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • CPR certificate required within 30 days of hire

Responsibilities

  • Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participant’s needs.
  • Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the community
  • Responds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordination
  • Provides education to participants so they can better understand the benefits of services and community supports.
  • Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.). Explains eligibility requirements, coordinates transportation for participants and other basic needs of support
  • Document all consumer interaction and maintain timely reporting and documentation requirements
  • Observes participants behavior; reports significant changes to supervisor or team; documents appropriately
  • Daily case conference with CMT about shared participants
  • Performs other related duties as assigned.
