The Social Work Care Manager I utilizes clinical expertise to perform psychosocial assessments, develop and implement care plans in collaboration with the appropriate care team, and assess crisis situations to provide clinical counseling, diagnosis, brief therapeutic interventions, and necessary resources or referrals. This role also includes providing individual and family treatment as indicated. The position works collaboratively with patients, their support persons, healthcare providers, insurers, community resources, and all other involved parties. This role will work with interdisciplinary teams to help pediatric patients and their families live the healthiest lives possible. As part of this role the Social Work Care Manager will complete psychosocial assessments, provide brief therapeutic interventions, connect to community resources, address mental/behavioral health needs, coordinate care with medical providers, and create plans of care with families. The SW Care Manager will have time scheduled in clinics including the Neuromuscular Clinic part of the week each week, as well as unscheduled time to respond to and support clinics without embedded SW Care Management.
