About The Position

The UW Medical Center-Montlake Outpatient Oncology Department has an outstanding opportunity for a Social Work Assistant. As a member of the Ambulatory Oncology Team, the Social Work Assistant position provides clinical and administrative support to a team of outpatient oncology social workers. Assist with case management, resources, and referrals to internal and external community services based on patient needs. Ideal candidates have experience working in a medical setting, demonstrate strong multi-tasking and organizational skills, oral and written communication, and computer proficiency. Must work well with a variety of persons and teams and possess excellent customer service skills.

Requirements

  • Bachelor of Social Work or a Bachelor’s Degree in the behavioral sciences AND two years experience in the social work field.
  • OR an Associate of Arts Degree in the behavioral sciences AND four years of work experience in the social work field.
  • Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration.

Responsibilities

  • Be comfortable working directly with patients and families in active cancer treatment.
  • Manage a referral pool and respond to patients who screen positive for SDOH (Social Determinates of Heath) needs on the “Supportive Cancer Care Screening” given to all new oncology patients.
  • Collaborate with the social work and medical teams on patient needs as they are identified during treatment.
  • Develop knowledge, update resources and provide patient education on oncology specific resources and programs.
  • Assist patient/family with basic community resources and assist with applications as needed for transportation, Medicaid lodging, UW financial assistance, oncology grant programs, etc.
  • Work with fiscal specialist to manage and track the “Oncology Donor Fund” grant program.
  • Attend interdisciplinary team meetings as needed for covered oncology clinics as well as SW&CC meetings.
