About The Position

This role involves assisting the team in delivering patient care, considering age and developmental levels, under the direction of professional staff. The Social Work Assistant performs tasks as specified by leadership, maintains an orderly patient flow, and adheres to safety and infection control policies. They also perform related clerical duties to ensure continuity of patient care, including utilizing computer technology, entering data, discharging patients, making referrals, and maintaining a safe environment. Effective communication techniques are essential, demonstrating a professional, caring, and respectful approach with patients, families, and co-workers, while understanding and respecting cultural differences and maintaining confidentiality. The position requires adherence to hospital policies and procedures, participation in staff meetings, and support for the hospital's mission and philosophy. There is an expectation for participation in the development of staff members, meeting health assessment requirements, identifying learning needs, and other assigned duties. Specific responsibilities include providing information on outpatient substance programs, connecting patients with resources like housing, food, and legal services based on Social Determinants of Health (SDOH), referring to community agencies and Health Home, calling patients discharged from the ED, and assisting inpatient Social Work staff with patient resources. Proficiency in EMR (EPIC) for documenting patient care navigator notes and knowledge of email/telephone etiquette are also required.

Requirements

  • Associate degree at minimum
  • Previous experience in a medical setting or 2 years relevant work experience preferred
  • Excellent communication/ interpersonal skills
  • Knowledge of computer technology preferred.
  • Successful completion of St. Barnabas Hospital orientation.

Responsibilities

  • Promote a safe, cooperative and professional health care environment to ensure optimum patient care.

  • Assist the Team in the delivery of the patient care considering age/development level, under the direction of the Professional staff.
  • Perform tasks as specified by Director of Care Transitions leadership team.
  • Maintains an orderly flow of patients.
  • Follows OSHA standards and Infection Control policies.
  • Seeks direction and supervision, when necessary, but functions independently where appropriate.
  • Correctly identifies all patients by dual identifier prior to performing any procedures.
  • Demonstrates flexibility with changing assignments due to staffing needs and adequately manages workload.
  • Performs related clerical duties to ensure continuity of patient care.
  • Utilizes computer technology in the work area.
  • Enter data accurately.
  • Discharges patients and make referrals as indicated by the team.
  • Maintains a safe environment for patients and report unsafe condition to the unit manager.
  • Utilizes communication technique effectively.

  • Modifies approaches based on age-specific needs and responses to treatment.

  • Provide information to outpatient substance programs.

  • Referral to community agencies. Meals on wheels.
  • Referral to Health Home.
  • Call patients that were Discharged from the ED.
  • Asst inpatient Social Work staff without patient resources.
  • Documents patient care navigator notes.
  • Respond to incoming emails and calls in a timely matter.
  • Ensure out of office notifications are activated on scheduled days off.
  • Review emails and voicemails on a daily basis.
