About The Position

The Social Work and Behavioral Health Manager for PACE provides clinical and administrative supervision to the PACE Social Workers, Community Resource Specialists and Behavioral Health Therapist. The person in this role manages the functions of the social work department which includes hiring and orientating new staff, performance evaluations, establishing and updating caseloads and reviewing Social Workers’, Community Resource Specialists’, and Behavioral Health Specialist’s documentation. The review of documentation entails verifying documentation is compliant with regulatory standards and ensuring proper standards of Social Work practice are being implemented. The Manager also maintains their own caseload which consists of performing assessments, counseling PACE participants, family/caregivers and providing case management.

Requirements

  • A Master’s degree in Social Work with current LICSW licensure in the Commonwealth of Massachusetts preferred (or LICSW within 6 months of employment start).
  • A minimum of 2 years of clinical and supervisory experience in the social work field; more than 2 years highly preferred.
  • One-year experience working with the geriatric population is ideal.
  • Bilingual skills are a plus.
  • Reliable source of transportation preferable.

Responsibilities

  • Hire, train, evaluate, and supervise social work staff, which includes MSW level social workers, community resource specialists, and the behavioral health specialist.
  • Collaborate and coordinate with contracted chaplain to ensure delivery of pastoral care for PACE participants as needed.
  • Participate as part of the PACE Management Team.
  • Assign caseloads to Social Workers and ensure proper collaboration between Social Workers and Community Resource Specialists.
  • Ensure departmental compliance with regulations and best practices. Responsible for reporting on social work-related PACE Quality indicators.
  • Coordinate quarterly PACE Ethics committee meetings.
  • Develop and review departmental Policy and Procedures.
  • Serve as department lead in creating, implementing, and evaluating quality improvement projects for the Social Work department.
  • Establish and maintain ongoing community partnerships, including Boston University School of Social Work, Ethos, various contracted Assisted Living Facilities and Skilled Nursing Facilities.
  • Provide consultations to the Interdisciplinary Teams (IDT’s) relative to psycho-social issues and participate in the IDT meetings as part of the team.
  • Provide psychosocial assessment, diagnosis, evaluation, crisis intervention, and treatment for a diverse caseload of patients and families, addressing a broad range of medical and psychiatric conditions
  • Complete all responsibilities as required of Social Workers.
  • Perform all other duties as assigned and/or required.
  • Must be able to travel between Upham’s PACE sites in a timely manner.
