About The Position

The employee will be required to sustain numerous duties which include the following: supervision of individuals, monitoring of self-administration of medications, education on medications with individuals, assist them on how-to call-in medications, how-to pick-up medications, and pay for their own medications, complete community skills and community meetings with individuals each week. The employee will also be responsible for day-to-day home maintenance, vacuuming, dusting, assist individuals with cooking, cleaning, laundry, teaching them how to budget funds, and education on good personal hygiene. The employee may have to physically help individual in and out of the shower/tub, physically demonstrate to individual how to bath and lather correctly. The employee will provide direct care to residential individuals, they will document daily on goals for each individual, assist in leading community skills meeting, and community skills training. The employee will be required to transport to day services, all medical appointments, psychiatric appointments, and any other pertinent appointments for the individual. The employee will be responsible for helping individuals on and off the van as needed. The employee will make sure to complete all Middle Flint Health & Wellness trainings and maintain trainings annually. The employee will make sure to be able to assist with indoor/outdoor activities, menus, completing monthly drills, and other duties as assigned. The employee will be responsible for physical activities which such as: assisting individuals on and off the van, assist individuals with walker or other manual devises. The employee will be able to shop physically be able to lift, carry groceries and supplies. The employee should be able to physically work with individuals on healthy activities, ie walking, working out at the gym. The employee will assist the individual, link them to services and resources, coordinate care, work along with family and/or support system in regard to their treatment, mental health recommendations and educate individual diagnosis and medications.

Requirements

  • High School Diploma, Certified Nursing Assistant, or associate’s degree in or around the related field degree in a social service-related field from an accredited college or technical school
  • At least 6 months to two years’ experience in a social service-related position OR One year at the lower level (SST010) or position equivalent
  • Must be at least 21 years of age
  • Have a valid driver’s license
  • Able to pass the criminal background check
  • Able to drive and transport individuals in a 9-12 passenger van
  • Satisfactorily complete all training requirements for Middle Flint Health & Wellness and DBHDD
  • Pass the ModivCare training and become ModivCare Certified Driver
  • Any male applicant between 18 and 26 years of age must present proof of having registered with the Selective Service as required by federal and state law, or of being exempt from such registration.

Nice To Haves

  • Bachelor’s degree is preferred qualification related to Mental Health, Psychology, and Sociology.
