About The Position

Sunshine Terrace Foundation is seeking a compassionate and organized Social Services Resident Advocate to join our interdisciplinary team. This position supports residents and their families by providing psychosocial support, promoting resident rights, assisting with discharge planning, and coordinating social services to enhance each resident's quality of life. Working under the direction of the Social Services Director, the Social Services Resident Advocate helps ensure person-centered care while maintaining compliance with CMS regulations, state and federal requirements, and facility policies. The ideal candidate will be a student currently pursuing their education in Social Work.

Requirements

  • High school diploma or equivalent.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work collaboratively in a healthcare environment.
  • Ability to maintain confidentiality and exercise professional judgment.
  • Basic computer proficiency.

Nice To Haves

  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Gerontology, or a related field.
  • Experience in skilled nursing, long-term care, rehabilitation, healthcare, social services, case management, or behavioral health.
  • Knowledge of resident rights, discharge planning, CMS regulations, and MDS processes.

Responsibilities

  • Advocate for resident rights, dignity, independence, and quality of life.
  • Provide emotional support and assist residents and families with psychosocial needs.
  • Participate in resident assessments, care planning, and discharge planning.
  • Assist with MDS/BIMS assessments when trained and authorized.
  • Coordinate referrals to community resources, behavioral health services, and financial assistance programs.
  • Educate residents and families regarding available resources, resident rights, and advance directives.
  • Maintain timely, accurate documentation in the electronic medical record.
  • Collaborate with nursing, therapy, activities, dietary, physicians, and other members of the interdisciplinary team.
  • Participate in quality improvement initiatives and regulatory survey activities.
  • Maintain compliance with HIPAA, CMS regulations, and facility policies.
  • Report concerns related to resident safety, abuse, neglect, or exploitation in accordance with facility policy.
