About The Position

The Social Services Program Specialist I supports MaineCare members with disabilities by coordinating services, resolving complex healthcare, financial, and placement needs, and facilitating collaborative care planning. This role works closely with community agencies, hospitals, providers, and care teams to ensure members receive appropriate services in the least restrictive setting while maintaining compliance with program requirements and client rights.

Requirements

  • Six (6) years of combined education and/or professional experience in one or more of the following: Social services casework, Social services program development or service delivery, Social services‑related fiscal or budget administration/analysis.
  • Licensed Social Worker (LSW)
  • Active license issued by the Maine State Board of Social Worker Licensure
  • License must be maintained throughout employment
  • Knowledge of current standards of care and practices related to healthcare facilities, service providers, and practitioners.
  • Knowledge of Office of MaineCare Services programs, policies, procedures, and guidelines.
  • Knowledge of client rights, confidentiality, and ethical standards.
  • Knowledge of community resources, agencies, and support services.
  • Ability to communicate clearly and effectively, both verbally and in writing.
  • Ability to collect, analyze, and interpret case material and make independent, sound decisions.
  • Ability to establish and maintain collaborative working relationships while maintaining professionalism and objectivity.
  • Strong organizational and documentation skills.
  • Applicants must be authorized to work in the U.S.
  • This position is not eligible for work authorization sponsorship (including H-1B sponsorship) by SSO and under the federal laws relating to the F-1 STEM EAD program, this position is not eligible to support a STEM EAD extension application.

Responsibilities

  • Provides consultation and problem-solving support to Maine residents with disabilities addressing complex healthcare, financial, and placement challenges.
  • Facilitates interdisciplinary team meetings for adults without a discharge plan until safe placement or resolution of complex issues is achieved.
  • Documents team meeting outcomes and interventions and updates records in the Care Coordination Unit (CCU) database in accordance with established protocols.
  • Assists members and care teams in accessing MaineCare services, community resources, and supports to promote independence and appropriate care placement.
  • Collaborates with healthcare providers and community partners to develop and support individualized plans of care.
  • Maintains effective working relationships with hospitals, providers, and community agencies to promote education, coordination, and optimal member outcomes.
  • Assesses eligibility and appropriateness for available services and waiver programs, including but not limited to: Brain Injury Waiver (BIW), Developmental Disabilities (DD) services, Adult Protective Services (APS).
  • Ensures coordination of services aligns with client rights, confidentiality standards, and departmental policies.
  • Performs other duties as assigned to support Office of MaineCare Services operations.
