About The Position

The Social Services coordinator works closely with residents and their families to ensure all matters of resident care are coordinated from admission to discharge. The ideal candidate is compassionate, organized and skilled at connecting individuals with community resources, support services and interdisciplinary teams to promote positive outcomes and improve quality of life for those served. They should demonstrate problem solving skills, maintain confidentiality and effectively manage multiple cases or responsibilities in a fast-paced environment.

Requirements

  • Bachelor’s degree in social services or related field eligible to qualify for WV licensure.
  • Licensed in West Virginia Social Work (LSW) – Required
  • Ability to complete all annual state-mandated training requirements, including regular in-service education and external training programs.
  • Strong knowledge of spreadsheet and word processing software for creating reports and correspondence.

Nice To Haves

  • Minimum of five (5) years of related experience and/or training in a geriatric care facility preferred; or an equivalent combination of education and experience.
  • Experience in long-term care, senior living, rehabilitation, or related healthcare setting preferred.
  • Knowledge of state and federal healthcare regulations and survey processes.
  • Strong leadership, organizational, communication, and interpersonal skills.
  • Ability to work collaboratively across departments and support a team-centered culture.
  • Skills specific to communicating with geriatric residents and their families are needed.
  • May need additional skills with special-need geriatric residents, such as those with dementia, Alzheimer's, or those requiring advanced health care.

Responsibilities

  • Effective leadership that includes supporting core values, addressing zero tolerance behaviors and maintaining active communication with all employees.
  • Reviews and prioritize grid of organizational and Minimum Data Set (MDS) assessments with various staff members each day including, but not limited to sections C, D, E, and Q of the MDS.
  • Reviews charting and data regarding residents' cognition, psychosocial information, behaviors, and moods.
  • Interacts daily with residents and their families to counsel, intervene, and report on action items including but not limited to: admissions, room changes, potential payer changes, resident life issues or concerns (can be walk-ins or scheduled in advance).
  • Coordinates and completes insurance authorizations and updates for various managed care and commercial insurance.
  • Complete and maintain PASRRs, HENS, and other documents required for SNF admission and/or continued stay
  • Complete and maintain NOMNCs and SNF ABNs as required
  • Coordinates discharges from the Orchards including, but not limited to making needed referrals, transportation, doctors’ orders (with nursing), pharmacy needs, and family meetings as needed.
  • Coordinates, manages, and evaluates facility concern and grievance programs
  • Attends meetings with other staff to discuss residents and to participate in care planning, restorative care planning, discharge planning, utilization review, falls, obtain/order DME when appropriate, case management, alleged abuse investigations, and quality assessment activities and analysis.
  • Attend other meetings as required or appropriate for position.
  • Serves as resident and campus liaison for SNF, ALF, and ILF with APS, the Ombudsman, and other resident stakeholders or groups
  • Travels to other campuses as required to provide social services support as required.
  • Directly supervises Social Service Coordinators and carries out supervisory responsibilities in accordance with the organization’s policies at the OEL and SP campuses.
  • Assist with performance improvement initiatives and continuous quality improvement efforts.
  • Support survey readiness/compliance activities.
  • Serve as a resource for regulatory standards, case mix management, and quality measures.
  • Participate in resident care problem-solving conferences and interdisciplinary collaboration.
  • Support initiatives that improve clinical outcomes, operational systems, and resident safety.
  • Maintain current knowledge of federal, state, and local regulations.
  • Promote an environment focused on resident safety, risk reduction, and error prevention.
  • Demonstrate knowledge of proper identification, reporting, and resolution of healthcare errors.
  • Collaborate with department supervisors to maintain compliance with organizational standards and regulatory requirements.
