About The Position

This position is for a Licensed Clinical Social Worker (LCSW) in an acute care setting. The role requires experience in discharge planning and care coordination, with a strong preference for candidates with diverse Case Management experience across various units such as Med Surg, LLOS, and ED. The position is at a Level II Trauma Center with 571 total staffed beds across two campuses (Colby and Pacific Campus). Charting is done in EPIC.

Requirements

• Master of Social Work (MSW)
• Washington State LCSW license
• Acute care experience
• Previous travel experience
• Epic experience
• BLS certification
• 1 year of experience required
• Must have Acute Care Experience
• Discharge Planning/Care Coordination Experience
• WA license required

Responsibilities

• Provide discharge planning and care coordination.
• Work in all areas of the hospital, including Colby and Pacific Campuses.
• Float between like units as needed.
