About The Position

Montebello Care Center is seeking a compassionate, organized, and resident-focused Social Services Director to join our leadership team at our 99-bed skilled nursing facility in Montebello, California. The current Social Services Director is being promoted from within, which means the incoming candidate will benefit from a smooth transition, strong support, and hands-on training from someone who understands the role, the residents, and the facility culture. Montebello Care Center is a 99-bed skilled nursing facility offering short-term rehabilitation, post-acute care, and long-term care services. The Social Services Director is responsible for supporting the emotional, social, psychosocial, and discharge planning needs of our residents. This role works closely with residents, families, nursing, therapy, admissions, case management, and the interdisciplinary team to ensure residents receive coordinated, compassionate, and person-centered care. This position plays a key role in resident satisfaction, care planning, discharge planning, family communication, and overall quality of life.

Requirements

  • Experience in a skilled nursing facility, post-acute care, healthcare, case management, or social services setting
  • Strong communication and follow-up skills
  • A warm and professional approach with residents and families
  • Strong organizational skills and attention to detail
  • Ability to work well with an interdisciplinary team
  • Knowledge of care planning, discharge planning, resident rights, and psychosocial documentation
  • A positive attitude and willingness to be part of a growing team

Nice To Haves

  • Prior experience as a Social Services Director or Social Services Assistant in a skilled nursing facility
  • Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or a related field preferred
  • Knowledge of skilled nursing facility regulations preferred
  • Experience with PointClickCare or electronic medical records preferred
  • Bilingual English/Spanish is a plus, but not required

Responsibilities

  • Complete social service assessments and documentation in accordance with facility policy and regulatory requirements
  • Participate in care plan meetings and interdisciplinary team meetings
  • Support residents and families with adjustment to the skilled nursing facility environment
  • Assist with discharge planning and safe transitions home or to the next level of care
  • Communicate regularly with families, residents, physicians, hospitals, and outside agencies
  • Help address resident concerns, grievances, mood, behavior, and psychosocial needs
  • Coordinate community resources, appointments, and supportive services as needed
  • Maintain accurate documentation in the medical record
  • Work closely with nursing, therapy, admissions, and administration
  • Promote resident dignity, independence, rights, and quality of life
  • Support a positive, service-focused culture throughout the facility
