About The Position
The Social Services Director at Autumn Aegis in Lorain, Ohio, develops one-to-one relationships with residents and families and helps coordinate social, health, and community services. Requirements
- Two years of experience in long-term care.
- Excellent communication and organizational skills.

Responsibilities
- Develop one-to-one relationships with residents and families.
- Establish relationships with area hospice agencies.
- Act as a liaison with social, health, and community agencies.
- Coordinate with the Activity Department to meet residents’ socialization needs.
- Oversee coordination of referrals for ancillary services, including dental, eye care, podiatry, psychology, psychiatry, and audiology.
- Assist with the admissions and discharge process as needed.
