About The Position

Warsaw Meadows Care Center is seeking a Social Services Director to join our nursing home in Warsaw, IN. The Social Services Director serves as a liaison and advocate for residents’ social and emotional needs and is responsible for planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department.

Requirements

• Bachelor’s degree from an accredited school in Social Work or a related field preferred
• If not licensed, must have a Social Service Designation in Long-Term Care
• Knowledge of social services practices, procedures, laws, regulations, and guidelines governing long-term care
• Strong communication, organization, compassion, and interpersonal skills

Responsibilities

• Develop social assessments and care plans that identify medically related social and emotional needs, with realistic goals and specific actions to be taken
• Participate actively in the pre-admission and admissions processes
• Listen and respond compassionately to residents and family members
• Work independently while maintaining professional standards and job-related responsibilities
• Document residents’ behavioral and psychosocial status
• Provide individual and group support to residents and families during periods of adjustment, crisis, or other times of need
• Establish and maintain a volunteer program designed to meet specific social service needs
• Orient residents and families to the nursing center and assist them in adjusting to their new environment
