About The Position

Sunnyvale Gardens Post Acute is seeking a dynamic, compassionate, and experienced Social Services Director to lead our department. Requirements

Bachelor’s degree in Social Work (BSW) or a human services field is required.
Minimum of 2–3 years of social services experience in a skilled nursing facility (SNF), post-acute rehab, or long-term care setting.
Proven experience managing a department or supervising staff within a healthcare environment.
Proficiency with electronic health record (EHR) software (e.g., PointClickCare) and regulatory documentation.

Nice To Haves

Master’s degree in Social Work (MSW) is highly preferred.

Responsibilities

Oversee the day-to-day operations of the Social Services Department, providing guidance and supervision to assistant staff.
Conduct comprehensive psychosocial assessments, develop individualized care plans, and participate in interdisciplinary team meetings.
Manage complex discharge planning processes, ensuring safe, seamless transitions of care back into the community or lower acuity settings.
Maintain full compliance with federal, state (CDPH), and local regulations governing long-term care social services.
Act as the primary liaison and advocate for residents and families, addressing grievances, rights concerns, and financial resource guidance (Medi-Cal/Medicare).
