About The Position

The Social Work Assistant will work with residents in the nursing home by assisting and supporting the social service department in identifying psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided in accordance with the National Association of Social Workers (NASW) Code of Ethics along with strict adherence to government regulations. The nursing home social work assistant shall support in providing residents with the highest practical level of physical, mental and psychosocial well-being and quality of life.

Requirements

  • High school graduate or G. E. D. equivalent
  • Ability to read, write, speak, and understand the English language.
  • Must have strong communication skills.
  • Must have the ability to make independent decisions when circumstances warrant such action.
  • Must be able to deal tactfully with team members, other staff, residents, family members, visitors, government agencies, and the general public.
  • May need basic computer literacy and skills.
  • ATTEND AND PARTICIPATE IN SCHEDULED TRAINING, EDUCATION CLASSES, AND ORIENTATION PROGRAMS TO MAINTAIN AND ENHANCE QUALITY OF CARE and ADHERE TO STATE AND LICENSING REGULATIONS.
  • Mandatory attendance at all scheduled supervision as required by state regulations.
  • Attend all required in-service and training programs required within your department.
  • Attend any mandatory meeting as required by your center.
  • Ability to move (sit, stand, bend, lift) intermittently throughout the workday.
  • Ability to lift, push, pull, and move a minimum of 50 pounds.
  • Ability to function independently and have flexibility, personal integrity, and ability to work effectively with staff and support agencies.
  • In good health and demonstrating emotional stability.
  • Ability to cope with the mental and emotional stresses of the position.
  • Must be able to read, write and speak the English language in an understandable manner.
  • Ability to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position can be fully met.

Nice To Haves

  • 1-2 years health related experience and skilled nursing facility or long term care preferred
  • Prior experience preferably with related software applications

Responsibilities

  • Assists the social services department in duties to support the overall functioning of the department with adherence to all Federal and State Regulations.
  • Perform general social services support duties at the direction of the Director of Social Services and/Social Worker.
  • Develops and maintains a good working rapport with other direct care givers to assure the residents’ psychosocial well-being.
  • Reports all grievances and complaints and make necessary oral/written reports to the Director of Social Services/designee.
  • Serves as the resident’s advocate working cooperatively with direct care giving staff to assure that the resident’s needs are being met and is aware of any changes in a resident’s condition and report immediately to the Director of Social Services and charge nurse.
  • Maintains open communication and provide information to the Director of Social Services that would improve the quality of services and/or activities and increase a variety of opportunities and choices for the resident.
  • Maintains confidentiality of all resident/family information at all times.
  • Maintains clean office area, organized and presentable at all times in order for you to be able to meet with resident families and other service providers.
  • Participates in departmental and center meetings and projects as directed by the Director of Social Services.
  • Participates in all projects assigned by the Director of Social Services.
  • Maintain a professional appearance, manner and approach while on duty.
  • Attend and participate in scheduled training, education classes, and orientation programs to maintain and enhance quality of care.
  • Attend all required in-service and training programs required within your department.
  • Attend any mandatory meeting as required by your center.
  • Perform other related activities as assigned or requested.
  • Maintain and work within established departmental, center, and home office policies and procedures, objectives, quality improvement program, and safety, environmental and infection control standards.
  • Promptly reports any suspected resident abuse, neglect or misappropriation to the Director of Social Services/DON.
