About The Position

River Oaks of Minnesota is seeking a full-time Social Services Designee to support the psychosocial, emotional, and behavioral health needs of residents within our assisted living community. This role is for individuals passionate about supporting, advocating for, and empowering residents to maintain dignity, independence, and quality of life in a meaningful role where they can make a real impact every day.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred
  • Experience working with individuals with mental illness, behavioral health needs, or in assisted living/long-term care settings strongly preferred
  • Strong understanding of mental health diagnoses, symptoms, and interventions
  • Ability to de-escalate crisis situations and manage challenging behaviors
  • Excellent communication, organization, and interpersonal skills
  • Ability to work collaboratively with multidisciplinary teams

Responsibilities

  • Conducting psychosocial assessments to evaluate resident emotional and social needs
  • Providing emotional support, active listening, and crisis intervention when needed
  • Advocating for resident rights, preferences, and person-centered care
  • Developing and updating individualized service plans focused on mental health and social needs
  • Coordinating with families, guardians, mental health providers, and outside agencies
  • Monitoring changes in resident mood, behavior, and functioning
  • Supporting behavioral interventions and care planning initiatives
  • Assisting with admissions, transitions, and discharge planning to ensure continuity of care
  • Maintaining accurate documentation in compliance with facility policies and state regulations
